Properly-enforced safety procedures would have prevented the death of a dock worker crushed by a container, an inquest jury has concluded. Mieczyslaw 'Mitch' Siwak, 34, from Bognor Regis, was killed at Flathouse Quay, Portsmouth, in 2017.
