Jeremy Warner represents family at the inquest into Miles Robinson’s death

The inquest examined the circumstances surrounding the death of Mr Robinson on 19 December 2022. A 999 call was made by his granddaughter at 03:37 but significant demand on the London Ambulance Service led to delays in the call being answered and in the allocation and dispatch of an ambulance.
The call was incorrectly triaged and assigned a category 3 (urgent) response, rather than a category 2 (emergency) despite Mr Robinson reporting symptoms that he believed were consistent with a heart attack.
After being told that an ambulance was not available, Mr Robinson travelled by Uber to a nearby Urgent Treatment Centre when he suffered a cardiac arrest due to a myocardial infarction. He was resuscitated but suffered two further arrests at hospital and sadly died shortly after. After hearing evidence from an interventional cardiologist, the coroner concluded the death was due to natural causes and the delays were not causative.
The Coroner, however, issued a Prevention of Future Deaths report, identifying a risk of future deaths stemming from the rigidity of the triage tool used by the London Ambulance Service. It was found that the system does not allow non-clinical call handlers to upgrade the response unless certain structured questions are answered in a specific way. The coroner warned that patients in similar situations may suffer cardiac arrests before help arrives, particularly when they are alone and unable to call back. If a patient is on their own, the cardiac arrest may cause their death prior to a category 2 (or subsequent category 1) ambulance arriving at their location.
